APPLICANTS FOR A HAWAII PHARMACIST LICENSE/EXAM AND PHARMACY INTERNS MUST ALSO SUBMIT PROOF OF DATE OF BIRTH (DOB) WHEN FILING YOUR APPLICATION, i.e. government issued I.D., birth certificate, or signed statement indicating your name and DOB. WITHOUT THIS INFORMATION, WE WILL NOT BE ABLE TO PROCESS YOUR APPLICATION.

CONTINUING EDUCATION RENEWAL REQUIREMENTS
- All Licenses regardless of issuance date, are subject to renewal by December 31 of every odd-numbered year.
- Licenses not renewed by license expiration date are unlicensed and may not practice.
Restoration Information (Restore within 3 years after license expiration)
Forfeited licenses may be restored within 3 years and subject to meeting restoration requirements. Failure to restore a forfeited license within 3 years after the license expiration date will result in the termination of the license and the person must apply as a new applicant and meet current licensing requirements.
