IMPORTANT ANNOUNCEMENT

For All Nurse License Applicants (LPN, RN, APRN and Prescriptive Authority) by Exam, Endorsement OR IF Renewing, Restoring, Reactivating OR Reinstating a Nurse License

Criminal History Record Check Requirement
Effective July 1, 2017

ONE YEAR (1) ABANDONMENT OF APPLICATIONS

Pursuant to HRS §436B-9 Your application shall be considered abandoned, will be destroyed, if you fail to complete the license process within one year after filing the application, or fail to take and pass the examination after becoming eligible to take the examination. If an application is deemed abandoned the applicant shall be required to reapply for licensure and comply with the licensing requirements at the time of the reapplication.

Use this option only if you do NOT plan to apply for a permanent RN/LPN license. You are only eligible for a temporary permit if you already possess a valid and unencumbered RN or LPN license in another state, territory, or country AND have already been appointed or accepted employment with a single health care entity in the State, as defined by Hawaii Revised Statutes Section 321-11(10).

This option allows out-of-state RNs and LPNs to apply simultaneously for a temporary nursing permit and a license by endorsement within a single application, for a single nonrefundable fee. You are only eligible for a temporary permit if you already possess a valid and unencumbered RN or LPN license in another state, territory, or country AND have already been appointed or accepted employment with a single health care entity in the State, as defined by Hawaii Revised Statutes Section 321-11(10).

IMPORTANT: This application must be submitted by mail or hand-delivered to the PVL office. It cannot be submitted through the online portal.

RN Nursing License

Advanced Practice Registered Nurse License (APRN)

If you are applying for a prescriptive authority in addition to the APRN license, please note that you will need to complete the prescriptive authority application.  Federal license portability provisions do not alter Hawaii’s prescriptive authority standards and do not extend another state’s prescriptive authority into Hawaii.  Applicants should not prescribe in Hawaii until Hawaii APRN licensure and prescriptive authority (and, when applicable, controlled substance registration) have been granted.

The application form for APRN Prescriptive Authority is not currently available on MyPVL.  You can download the form from this webpage or posted above.

  • If you are submitting the TMS application online with prescriptive authority on MyPVL, please upload the prescriptive authority application with your APRN TMS application and along with required documents.
  • If applying via mail (hardcopy), attach the prescriptive authority application with your APRN TMS application along with required documents.
